New Participants
If you’ve received an invitation to participate in WAVES

1. Read the Consent and/or Assent Forms.
2. Enter your survey access code online to take a survey in REDCap. This can be found in the letter or postcard you received in the mail.
3. Fill out the questionnaire.
4. Schedule a sample collection and get your gift card!

If you did not receive an invitation via mail or phone call, but want to participate:

1. Read the Consent and/or Assent Forms.
2. Click Get Started and submit an intake form in REDCap.
3. Look for an email from UW Medicine WAVESurvey (thewavesurvey@uw.edu).
4. Fill out and submit the survey questionnaire.
5. Schedule a sample collection and get your gift card!

Returning Participants
Thank you for participating in WAVES.

Once your results have been processed, you may access them in our secure web portal using the code provided at your visit.

View your test results.

Participants will be invited to test again during additional phases of the survey, up to three times total.
